My form is disabled

I have entered the embedded code and another code format and neither one shows the form on my site.  It says it is disabled.  Please enable this form and will you tell me how  to keep it from being disabled again!

I have activated your account. I would recommend you deleting the forms that are blank. 2 of your forms only has a title and a submit button.
