I attached the email confirmation. Dates and times should be showing up next to each description but instead it just says "Register .
Upon reviewing your form I can see you're using the Quantity Gift Registry to list your dates.
Do note that when submitting the form you will get the widget's label as well as the selected quantity:
Now, I can see you have set up custom labels in your email notification, this is why you may see the labels are different on the email than the submissions page, but the selected option will be sent like "Register", being "Register" the option selected and followed by the quantity user selected.
If you need to get the date, then you should use a different widget, here you could find different date picker widgets: https://widgets.jotform.com/category/pickers
