How to have a meeting room time slot usig the form

Hi Guys,

I am trying to implement a new idea with Jotform. Having used it for my first assignment, I was on cloud nine when things went live with my Manager

There is a meeting room in my office and as of now there are 5 teams. There is a Excel Sheet stuck on the outside of the door to the meeting which says – which team would be using that meeting room and the time slot they would be using it for

I was searching your site but could not find a pre – defined form for such a form

My idea is to get a form which intakes 5 logins (basically 5 team leads) – and then they would display the availability of the meeting room

This data would be shared only with the team leads and no one else

Could you guys please guide me to a form that has the framework I am looking for?

This widget will allow you to have a pick list like selection that have limit in number of slot available. 

You can put the room time slot in the description so the user can see which time/room are available.
