Can't get form working in Wordpress

Hi, we've got our site ready to go and want to launch it on Friday.

I am just working on adding three jotforms (it's a fundraiser, so we've got one each on three pages).

I'm using the Divi theme. First I tried the embed script, in a code module. It looks great but just says 'please wait' when you click submit order.  (example: http://friends-of-tyc.bravehost.com/fundraising/spolumbos/)

Here's an example of the publish > platforms >wordpress.org code
    http://friends-of-tyc.bravehost.com/fundraising/perogies/

Then I tried using the plugin, but it didn't link to my account and nothing came up after I clicked on the little pencil icon (I am logged in to Jotform on the same browser).

I tried using the iFrame code in a text module, but it still says 'please wait'. Eventually a little sad face icon appears in the place of the form.   (example; http://friends-of-tyc.bravehost.com/fundraising/greens/)

All are looking fine, they just don't submit.

    Thanks for your help.
