Cannot create fillable PDF copy of my form with payment field

Hello. I have tried a few times to create the fillable pdf but the wheels just keeps spinning for hrs saying its "working" with nor result. Here is my form.

Fillable PDF forms are much more limited than their online counter parts.  Elements such as widgets or in the case of your form payment fields, do not translate properly to fillable PDF form.  Removing the payment field from the form should allow for the PDF to generate properly.
