Our "New User Access and Maintenance Form" unexpectedly became disabled and stopped working. Do you know why this happened? Did we go over our submission limit?

I checked your account and it is in good status. I also opened your form and it is working fine on my end. No, you didn't reached the submission limit. Upon checking, you currently have 21 submissions.
