can this form be fixed?

Hi there!

I am unable to edit my forms

After clicking Edit Form from my list of forms, it only brings up several error messages. I've tried cloning a form, but the clone too produces the same errors.

Additionally, i'm unable to create new forms. The screen simply hangs from the new form modal window with the 'Creating form...' button grayed out.
