New form: Preview form says Form Not Found/disabled

Hi team,

I created my first form. When clicking on the preview switch on the top right corner, the page says:

Even though it's Enabled in Settings. My account is dave@tresorit.com - thanks.

I've checked your form and it has been disabled due to phishing suspicious, upon reviewing it I can see you're using words such as login and password, although you're not asking for such info on your forms it's not allowed to get these words in your form. 

In order to reactivate your account we will need to delete all those fields where password or login word is included. 

I would also suggest you to check our Terms of Use since there's stated what you cannot ask on your forms, this way you will also avoid getting suspended in the future.
