Facebook fitform intergration

Hi I created a fitform integration and it is not working. It says submit and looms like the the standard form.

You have been auto suspended due to the "Login" button in the form. Please modify this button, you can keep the default "Submit" text in it. 

As for the Facebook integration, I don't see the form integrated with Facebook. Please re-do the integration and make sure that your Facebook App has the correct domain name, as per the instruction in the following guide:
