I need an entry with single selection for my form

I'm trying to set up a form field that asks if your pain comes how often for as many many hours/days/weeks, where they would make an entry as to how often their pain would occur i.e. "3", then they would be able to make a selection of hours, Days or Weeks. Is there a widget that would allow me to do this?

If I understood your question correctly, you can achieve this with ordinary form fields. For example, you could have a Number field where the user would enter the amount, and have a Drop Down field where they could select the frequency:

These options to merge lines aren’t even available in version 3 or 4. How am I suppose to do this if the option isn’t available anymore?
