Form no longer works in 3rd party site

This form has worked for months and randomly stopped working now.

I am seeing extra code at the top. The scroll bar is jumping. And the bottom of the form keeps flashing.

Please note that we had an issue with and our developers have already fixed it.

You may check your form again and if the problem persists, just trigger a save by changing anything, and once the form is saved the problem will go away.
