As explained by my colleague. Due to the form https://www.jotform.us/form/42526796943165 ,
I had reactivated your account but you need to immediately delete the fields asking for Credit Card Information or you can delete the whole form. Your account might be suspended again if you don't do this immediately.
To Integrate payment option with the form please follow the instructions provided at https://www.jotform.com/help/323-Mastering-Payment-Form-Integrations-with-JotForm

How can I remove the form on the website?
I can not log into the website..

Pleas retry login to your account/username kendallreceptionhall again using this link https://www.jotform.com/login
Once logged in, delete the form/s mentioned that violates our terms.
Let us know if your are still not able to do this.
