If you create a form and send a link to people via email, can those individuals respond anonymously or is their name displayed?

In addition, do the people who are responding have to sign up have a Jot Form id in order to access the forms?

You can make the name field as an optional field so they can submit the form without the name field. Well, the IP Address is still recorded. Yes, the form ID or link is required for them to fill/submit it.
