Form does not work anymore and gives the form disabled error

My form worked fine before but now it does not work anymore and gives the following error:

Would you mind letting us know the use of the fields in the form? 

Yes, It was a password to get onto the form. the rest where field where poeple could post a message and a foto for the social media of my organisation. I changed it to secret, is it now oke??
