I'm trying to have people answer one question at a time and it looks like I'm messing up the conditioning function.

I want the first question to stand alone and then once answered, have the second question become visible and once answered, have the third question become visible. I thought I was doing this with the conditioning format but it's not working. My first question and third question are shown at the same time. Can you help me?

I would suggest you to use page break to achieve your goal.
