Can I create my own form

I need to create a checklist.  Can I do my own form

Yes, you should be able to start creating your own form by going to your My Forms page and click the Create Form button. 

A window will appear, just click the Create button under Blank Form.

It will go to the form builder with a blank canvas where you can add fields.
