We have been experiencing issues with files being attached to the form. We have reported this before but it has been happening frequently. Please see attached screen shot.

This problem could occur if the user had an intermediate connection while uploading or the upload was interrupted by some other reason, but the partially uploaded file will stay causing the user to be able to submit the form.
A solution would be to enable Multiple upload option for your upload field so your users are aware of the upload and can actually see the file finishing uploading:
Regarding your file, I'm afraid that it will need to be re-uploaded since the file wasn't completely uploaded by the user.
