Why my client account has been autosuspended?

    Hello Team,

    Just wanted to confirm with you about my client's Jotform account which has been autosuspended by you. Username for the account is "tailsaroundtownpet". Please check and resolve to make this account back in running mode as we have forms built on it which are running on my site and they are now showing error pages.

    Please fix ASAP!

    Account tailsaroundtownpet is autosuspended by our phishing system because of the Payroll Information Form. Inside of that form there is a password field and it is against our Terms of Use to collect login credentials such as password. 

    Phishing. You agree that Company may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

    I have deleted mentioned field from that form and I have activated account of your client.

    Please tell your client to check our terms of use and to not create similar forms.
