Narrative Fields: Why all fields have the same submitted data?

I have the following script in the Narrative Widget:
 Confirm your Club Orientation workout to get started right away!  Plan to meet with (Name){Text Box A:10} on (Date) {text box A:10} @ (Time) {text box A:10}.

#input_44  which has the unique name "orientation" duplicates the DATE entered in the TIME field as well when it sends the email and PDF copy.  Each field of the widget should display what is input.

I've checked your widget and I can see all the fields have the same names: 

Do note that as explained in the widget instructions (right below the template area), each field must be unique, otherwise your fields will generate conflicts.. 

Please, apply this change to your widget and let us know if the issue persists. 

Can you please give an example of the changes you are suggesting to make sure each field returns the unique response.

You should set your fields in the following way: 

{Name:10}

{Date:10}

{Time:10}
