Please remove all fields that is asking for a password on your form so we can activate your account. If you want we can also delete it and restore your account in good status. We just need a confirmation so we can login to your account and remove the guilty fields.

I checked your forms and there's still guilty fields asking for a password. I have deleted the fields asking for a password so I can re-activate your account.
