I'm getting a DNS error when accessing my form using its Custom URL

Created a custom URL for my form 

https://form.jotform.us/phillipspet/spring-buying-show-retailer

 

When I try to go to the URL I get the following error:

 

This site can’t be reached

form.jotform.us’s server DNS address could not be found.
