Gift Registry is not publishing with my form

Gift Registry is not publishing with my form. Everything else on the form publishes, but net Gift Registry.

Here is my form:

I just saw a yellow barred message that said the "This form will be hidden," but it does not say why.

OK, with more testing, the form does not hold a widget when published. I have created forms with widgets before, with no problems, but this one is not publishing them them.

Can some one help?

I have checked your JotForm and see that the Gift registry widget is displaying normally at my end. I have also cloned the form to my account and see that the widget is working fine. However, I see that the quantity is not provided in the widget for the options. If you are experiencing any issues with the widget, could you try updating the widget and see if that works?
