How to install a code to access my forms.

I ant to install a code to access my form, can someone explain this process, with screenshots please. I can see a lot of threads about this but find them all somewhat confusing. Many Thanks

I created a demo form and applied this Condition rules

As you can see on the conditional statement the access code is '123'
