Credit card slot not working on our form

There's nowhere to enter card # and ex. date.  Please help.  usere name: mdward80

I'm not sure I understand your question correctly, are you referring to a payment form? If so, please provide the form name/URL so we can look into it. 

Also please keep in mind that it's against our Terms of Use to collect sensitive information such as credit card details, without using one of our payment gateways.
