We would like our form design to look like the attached picture

    Hi, 

    we created a mockup of what we want our form design look like for social affiliate. You can see what it currently looks like by visiting the link, but we want it to look like the attached image. 

     

    Please advise.

    Considering the heavy custom CSS you introduced into the form, could you please see if adding this custom CSS code to the bottom would work for you?
