    How can I add multiple fields to a question?

    I'm sending out a form and getting parents to register their children for either a Teens or Kids Class....

    I want to set up the form to read

    Participant details:

    NAME     AGE     CLASS (TEEN OR KIDS)

    NAME     AGE     CLASS (TEEN OR KIDS)

    ...

    ...

    As some families have more than one kid/teen....

    I want it to look like this one
