My form that I have created is showing not found.

A form that you had imported requests passwords.  This is against our terms of use:

I removed the form and activated your account.  Your forms should be usable once again.  Please refrain from importing or creating any forms that request usernames or passwords as it will result in your account being suspended once again.

How funny.  I had no idea I did that or even how I did it.
