I can't remove the input background because nothing is showing in the "FORM BACKGROUND" textbox in the theme designer. Thanks!

In fact, the theme you used has a background image as you can see here but it does not attached to advance designer to allow you to be able to change or remove it.
I'm escalating this to our developers to fix it. Once the issue is resolved, you will be notified via this thread.

I was informed by the developer who was assigned to this ticket. Related theme used by you was updated. However, you need to re-apply the same theme to make changes take affect.
Could you please try and let us know if the issue still persists?

However, as far as I check, the issue with the background seems to be solved in your form as seen below:
