Checkbox's - if selected need a date field to be available for filling out a date

For every checkbox selected, I need to set a rule where if selected a date field is available for the patient to place a MM/YYYY information.  There are a multitude of checkbox's than can be selected and I want to the form to appear uncluttered.

Any ideas or suggestions would be greatly appreciated.  

I think this would be achieved by using Conditional Logic on your form. Smart Forms Using Conditional Logic.

You can set up a condition on your from where, if an option is selected from the Checkbox, the Date Field will be shown on your form. Please refer to this guide on How to Show or Hide Fields Base on User's Answer.
