My Form is not passing to my thank you page

HI There,

When filling out the Ultimate Life Application form and hitting submit... It acts like it is going to the thank you page.. I see it briefly and then it goes to a white screen and says Missing Next Page.

How do we fix this?

I checked your form and see that you have enabled Send Post Data option. This causes the redirection to fail. Please disable the option by selecting no under the form settings:
