Submission data for one of the question is not recorded.

We have a choice for lunch options but the user's choices aren't being recorded. How can we fix that?

I am not sure if I have understood your issue correctly. Can you please explain your question a bit more in detail?

Do you mean to say that submission data for "Lunch Options" payment tool is not being captured in submission. I did test your from's submission page and the data seems to have been captured correctly. Please check the screenshot below:
