Trying to send out a form but it wont stop showing to be disabledPage URL:

I have checked the status of your account and I see that it is on autosuspension status. This is what's causing your forms to show disabled whenever you access it. When I checked your account to find what triggered the suspension, I found out that there are forms in your account that have a field(s) or text(s) guilty of violating our terms.
Please note that collecting of sensitive information such as login passwords in our forms is prohibited.
We have an anti-phishing tool that scans every form and marks any form suspected of phishing activity then suspends the account that owns it after.
I have removed the said fields and texts from your forms and reactivated your account.

It's still showing as disabled for me- This is our internal company information. These are our company owned accounts. Not trying to hack anyone, just was hoping to collect their updated info more easily since their are over 60 different property accounts on our business manager. I will do it another way I guess. thanks.

As we understand that the information you are trying to collect will only be used internally (in your business), we would like to apologize, however, we also have terms and policies that we need to implement.
With regards to your form showing disabled again. The revision history of the form, http://www.jotform.us/form/70875560268162, shows that there are recent changes in the form that lead to it being marked again. I have deleted the guilty fields again from that form and reactivated your account.
