FORM ISN"T WORKING! Totally panicking, please help!

    This form is getting an oops error both on the jot form preview and through squarespace. Says OOPs, form not found:

    PLEASE HELP IMMEDIATELY! I just sent an email out to 100 people asking them to sign this!!!
