My form says not found

I have checked your account and your forms.Your forms contain Credit Card Number, Expiration Date fields etc. You can not directly get personal sensitive information such as password, credit card details. Because it violates our terms of use. If you insist on collecting any of this information, your account will be suspended again.  

I have deleted these fields and re-activated your account. Your forms are enabled now, you can log in to your account.
