I have received a few (several?, not sure how many…) e-book orders through JotForm & WooCommerce without the money for the orders going to PayPal or my bank account.
Any thoughts on where the customers’ money is going and how we can fix this?

I have checked your form and realized that your Payment field "Your Choice" is not required. So, people can submit the form without filled this field and not purchased for you.
So, these submissions from the users that submit the form without filling payment field. I can suggest you make required to your payment field.
