I keep getting the same form submitted multiple times

Hi,

I keep getting a notification letting me know that a new form has been filled out and submitted, but when I go to check it is the same one (The original one + 2 other forms). 

I checked your form submissions and it looks like the user simply submitted the form multiple times.  There are 3 submissions from the same user then 2 other unique submissions.
