Additional email if button is checked.

I need the ability to have another email sent to another email address plus my email if a button is checked. 

On my contact form, there is a question asking if the customer needs a certificate of insurance. If the customer selects yes, I need it to send an email to our insurance agency with the customer's name address and email. 

Is it possible to set something like this up?
