Why is only one submission per user being saved?

I just received a response to this question from Jan.  Thank you.  However,  it did not answer the question I asked.  I see the test submissions.  The problem is,  that one person submitted 5 entries and only one is in jotform  - an evaluation for Rachel  and the other person submitted 11 entries and only one an evaluation for William is there.

If you have received 16 submissions, you should also see this under your history. But I just checked it, you only received 7 submissions and 5 are our tests. Maybe those are submissions in the other forms? I would suggest checking further. You have 20 forms, I cannot check them individually this time.
