I checked your form and I can open it without problems, it's most likely a local network issue, can you please try from a different device, or different network?

I'm still not able to replicate, your form seems to be up and running, and there's no issue on our side.
Have you tired opening your form from a different network or a different device? If not, please try and let us know how you get on.
