Pricing for non-profit and conditional formatting

    Hi,

    We're a 501(c)3 needing to replace our Fluid Surveys forms. What is the non-profit pricing?

    Also, we use a conditional formatting and was wondering what payment plan would solve that need.
    We need to send the full response in an email to one of 5 email addresses based on one question. The user selects their "Account Manager," and we then want the full form sent to that person.

    We offer a 50% discount on subscriptions for non-profit organizations.  You can apply for the discount from any active account.  Here is how to do so:

    1. Make sure you are logged into the Jotform account you would like to add a subscription to.

     2. Then, fill out and send this form along with your organization info (site URLand PDF docs) for verification.

     Give us up to 24 hrs to process the request. Upon  approval, you should get an email like this:

     

     

     

     IMPORTANT: When you get the "Congratulations" email with the approval, before clicking in the "Upgrade" button in the email, make sure you have opened a fresh session at Jotform site with your account
