I keep getting an error message when I try to publish my form. It keeps telling me the form failed. How do I fix this problem?

I was able to reproduce the issue on your form and even on my clone version of the form as well.
I found that it was being caused by the e-signature widgets on the form.
I removed all the existing E-signature widgets on the form and replaced with Smooth signature widget and the form loads successfully.
Please try the fix on your form also. Let us know if it still did not resolve the issue.

After doing more test I found that removing and adding back the E-signature widget will also work. So you do not actually have to use a different signature widget to fix it.
Let us know if issue persist.
