Why check box fields are not included in submissions?

When the "bath before discharge","nail trim before discharge", "daily brushing" or "pro grooming" check boxes are ticked off-the boxes don't stay "checked off". They appear checked but once you submit the form-when you re-open it they are empty.
Makes the form unusable and has caused problems with these items not getting done. UGH!
Please advise asap.

I have cloned your form and I was able to see the same issue when submitted it on my end, the check boxes seem to be empty even on the submissions page. 

After further inspecting your form I noticed that you have not added an option on it, the check box fields only have their labels and an empty option. 

I would suggest you to please add at least one option to your form, this should be included in the submission.
