Why is my form disabled

    I RECEIVE AN ERROR MESSAGE "Form NOT Found." the form is disabled.

    Your account is suspended because one of your forms is asking for Credit Card authorization upload. Here's a screenshot:

    Also, one of your forms has a Header title Credit Card Charge Authorization.

    If you let us delete the Header Title and the Upload Cr Card Authorization then we can re-activate your account. As per our terms of use, collecting sensitive information is not allowed here in JotForm.
