Please help as my account got autosuspended

    Upon inspection I found that your account got autosuspended because your form "MyTimeMD Patient Sign Up Form" with ID 32866948429169 is collecting passwords, in violation of our terms of use.

    If you accept our terms of use, and grant us permission to remove the offending form, we may consider re-activating your account.

    I have now removed the guilty fields and activated your account back in good status. You can now use and manage your forms properly.
