Form Calculation not working

I have used form calculation widget to calculate the sum of ticket sales. I have followed the instructions from 'How to set up a payment order form with limited stocks' guide to pass the calculated total to the payment field; however after selecting 'user defined amount' in the payment type i do not get the the pop up message as in '3' above and the only options available to me in then'Get price from' field are 'None (manual)' or 'clickto37'. Can you advise why I cannot see the 'calculation widget' option? Thank you

I cloned your form to test it and I was able to replicate the issue. I made changes to the cloned form and it is now working properly.

I've switched the "Price" and "Product Name" to fix the Form Calculation Issue. After that, I have selected the "clickto37" in the Get Price From the drop down menu and clicked the Save button.
