date field blanks out upon return

when i go out of my form and then come back to fill it in again later, the date field is blanked out.  i'm trying  to create a simple timesheet that my employees can fill in every day and then go in and up date it.  at the end they put in their initials and the submit button appears to that they can submit it to me for my payment.  the problem is that when you close out the window and then go back in to edit, the hours all are still there but the date field goes back to mm-dd-yyyy instead of keeping the value entered before.  here is a link to the form. i have all of the math worked out but the date thing is daunting me
