Problem to instert a form in a wordpress web

The form can't be sent from the Web, but it works in Jot Form. Can you help me?

Can you tell us more details on what you mean by the form can't be sent from the web? I made a preliminary test on your form to see if it would show errors for trying to submit form with empty fields and it captured the warnings. So I assume that it should work fine when making an actual submission.

Also, is it alright if we try testing by making a submission to your form on your page directly? We would like to ask permission first because some do not like making submissions on their live forms.
