I hit I forgot my password and it says that I am a guest. I am the one who set up our forms so I don't know what to do.

The Page URL that you have provided belongs to the account username ucctolland and it is registered to the email address email@example.com
The email address is on the bounce list. That's the reason why you're not receiving the password reset instruction email.
