I have checked your account and see that it is currently holding Free subscription and created two forms in the account.
Please be noted that we have different subscription plans depending on your usage requirements that include Free subscription plan. On a Free subscription plan, you can create up to 5 forms, provides you with 100MB of storage space, stores 500 recent submissions. You can also receive up to 100 form submissions/1000 form views/10 payments in a month. If any of the monthly limits are exceeded in a month, the forms shall be disabled and you'll be required to upgrade your account or wait for the submission counter to reset on the first day of the following month. Also, Free subscription accounts shall be displaying JotForm branding banner on the banner. If you want to remove this banner, you'll need to upgrade your account to a paid subscription.
