Why my account is suspended?

Please help! My account is suspended, I don't know why?

I see that the account is suspended due to the form http://www.jotform.me/form/62748378228468 which is collecting sensitive bank information. Please be noted there are few guidelines for collecting bank account information. We request you to go through the thread below that can help you in making the changes to the form.

You may need to remove the fields on the form if you do not follow these guidelines. I have now activated your account allowing you to make necessary changes to the form. Please make sure that you make the changes immediately to avoid any further suspensions.
